Summer School OFF Campus Waiver

I understand that Toussaint L'Ouverture High School for Arts & Social Justice (TLHS) is located in downtown Delray Beach and Boca Raton, which does not allow the school to have a campus. Therefore, I am aware that when my child is given a snack break or lunch break that they may be leaving campus and I will not hold TLHS or any of its agents (i.e.: America's Learning Centers) liable for my child's actions.

I also understand that the teacher may bring my child, with other children on short field trips to the Library, Beach, Crest Theater, Museum, etc., for an educational experience.

I ____________________________ have chosen to register for Summer School on the campus of Toussaint L'Ouverture High School for Arts & Social Justice (TLHS). I have chosen to do this to improve my grades and my academic record. I recognize that TLHS Summer School has Rules and Regulations and I agree to adhere to these Rules and Regulations.  

1. I will come prepared (Pen, pencil, notebook, etc.) each day.
2. I will remain attentive in class and complete my work.
3. I know there is NO Guarantee that I will pass Summer School.
4. I will RESPECT – ALL Teachers, Staff & Property.
5. I understand that being Tardy will hurt my grade.
6. I understand that leaving school early may hurt my grade.
7. I understand that there are Computer Rules and I will follow them or loose my opportunity to go on the computers.
8. I understand that inappropriate behavior during school, breaks, lunch, and class trips or anywhere on school premises may result in dismissal with total forfeiture of funds and no grade.
9. Cell phone are to be turned off during class.

10. Walkman, iPods, MP3 Players, etc., are not allowed

during class, UNLESS the teacher has approved their use

for that particular time or day.
